Follow Up Visits for Established Patients:
- 25 minutes: $175.00

*New patient consults will be conducted at our clinic. Follow up visits can be conducted in-person, by phone, or through face-to-face telemedicine consults.
Nutrition Consults for Established Patients:
- 30 minutes: $75.00
- 60 minutes: $125.00

The annual membership is highly recommended as it takes focus off the financial aspects of the visits allowing sole focus to be placed on the healing process. We want you to be successful! With the cost of annual visits paid up front, it encourages routine follow up and a better therapeutic partnership. The annual membership includes a cost savings of over $600 a year which does not include the money you will save with the supplement discount. Our highest priority is helping you regain your health and we want to minimize as many obstacles as possible that could influence your journey to healing.
**To take advantage of the VIP Annual Membership you must purchase the package prior to or immediately after (on the same day) your initial visit.
GrassRoots Functional Medicine is a fee-for-service practice and does not accept medical insurance. At the end of your consultation, the clinic will provide you with an itemized bill with traditional ICD-10 codes and CPT codes and you may file that with your insurance company. We do not assist with insurance reimbursement above and beyond the diagnosis and test codes and the receipt given at check out. You may also use pre-tax dollars from a Health Savings Account (HSA) to pay for your consultations and lab fees.
