Dear Potential Patient,
Welcome to GrassRoots Functional Medicine. We look forward to becoming your partner in your journey to achieving optimal health! Before your first visit, please take a few moments to review our website explaining functional medicine. In your first visit, which is face to face, Dr. Osgood will obtain a complete medical history. The key to functional medicine is treating each person as an individual and getting to the root cause of health problems. This generally entails a detailed conversation about your current state of health, health history, family history, diet, lifestyle habits, etc. At that point, Dr. Osgood will discuss potential approaches to improving your health and will recommend laboratory workups that will be drawn the following day. This visit will last approximately 80 minutes and is primarily designed to gather information and discuss your health history. Dr. Osgood may make some simple recommendations at this time, but most advice will be deferred until after the lab results are reviewed, and there has been time to carefully consider your case. Although optional, it is highly recommended that you schedule a nutritional consultation with our Functional Medicine Registered Dietician two to three days after your initial appointment with Dr. Osgood to review the dietary aspects of your treatment plan. The nutrition consult can take place via phone or face-to-face video conferencing.
The second visit, which can be in person or via phone, is generally scheduled 4-5 weeks after the initial consult (50-75 minutes in length) and is to evaluate progress with dietary recommendations and to review the conventional and functional lab results ordered at the initial visit. This time also includes a discussion of what may be causing your health problems and what supplementation (vitamin, minerals, herbs), diet, and lifestyle changes may be needed, as well as any or other medications that may be appropriate for your care.
How often you see Dr. Osgood after the third follow up visit will depend on why you are being treated. Some healthy people see Dr. Osgood only once a year for physical exams and screening lab tests. Many people who have multiple complaints are seen more often based on the severity of their condition. If you have any further questions after reading the enclosed information, please call our office, and we will be happy to assist you. Please be sure to complete all of the required forms and submit them on the patient portal before your appointment. We look forward to working with you.
In good health,
GrassRoots Functional Medicine
